If you are in need of a prescription for an outside pharmacy, please let the doctor know at your visit so we can provide you with a print prescription form that you can take or send to the pharmacy of your choice. If you decide later that you would like a paper prescription for an outside pharmacy, we will provide you one for the prescription fee of $15 that covers staff time.
